pay stubs or other proof of income for the past full year (all household income from all sources, regardless of whether a joint bankruptcy case is being filed)

copies of all income tax returns (including all W-2s and schedules) for the past four years (including W-2s, 1099s, etc.)

most recent retirement account (401(k), 403(b), IRA, pension) statements and plan summaries for the past 12 months

bank account statements for the past 12 months

all bills, statements, judgments, collection letters, and other proof of debt, including current

mortgage and home equity loan balances and vehicle purchase contracts and leases

vehicle title(s) and all other titled items

recorded mortgage(s) for all real property owned during the preceding six years

recorded deed(s) to real property owned during the preceding six years
any other instruments of record

real property tax bill/state equalized value

proof of all insurance policies (including evidence of cash value for life insurance policies)

copies of lawsuits, judgments, and garnishments and information on any potential causes of action

judgment(s) of divorce

child support order(s)

any deed, account, safe deposit box, or titled item the debtor has an interest in or is listed as an

interest holder on

claims

gambling activity (inquire if the debtor is a gambler and/or had gambling losses or wins within the proceeding one year)

balance transfers

whether divorce is anticipated or contemplated

inheritance issues
